HOW TO ADD AN ADDITIONAL INSURED
- An additional insured can be added online when you purchase the team insurance. Complete the “Field Owner” section and we will send you a separate certificate for each field owner/event sponsor requested.
- You can add additional insureds by sending an e-mail to [email protected] with the following:
- Team Name
- Certificate Number (bottom left of certificate)
- Field Owner or Event Sponsor (name & address)
